Privacy and data use
This diagnostic may contain commercially sensitive information about an AI programme, business function or transformation context. DigitalXform treats diagnostic answers and reports as private assessment data.
What data is collected
The diagnostic collects your full name, email address, optional organisation and role, assessment context, answers to the diagnostic questions, generated scores, risk flags, insights, action plans and report metadata.
Why it is collected
Your data is used to create your Galapagos AI Execution Diagnostic Report, allow you to access past results, support deletion requests, and enable DigitalXform to follow up about the diagnostic result, relevant advisory support or related Galapagos resources.
Data retention statement
Diagnostic data is retained for up to 24 months unless you ask for an assessment to be deleted earlier. You can delete an assessment through the secure My Results portal.
Delete my assessment
Sign in to My Results, open your result history, and use the delete button beside an assessment. Deletion removes the assessment, responses and generated report from the live diagnostic database. If it is your only assessment, your user account and login tokens are also removed.
Follow-up use
DigitalXform may use your email address to follow up about your diagnostic result, discuss relevant next steps, or share related Galapagos AI Execution resources. Your diagnostic data should not be used for unrelated marketing lists without additional consent.
